Thyroid Peroxidase Antibodies (TPOAb)
Present in most cases of Hashimoto's thyroiditis
Thyroglobulin Antibodies (TgAb)
Often elevated in Hashimoto's thyroiditis and thyroid cancer
TSH Receptor Antibodies (TRAb)
May be stimulating (as in Graves' disease) or blocking (causing
hypothyroidism)
Early detection of these antibodies allows for proactive
interventions to modulate immune function and potentially
prevent progression to overt thyroid disease [11].
Thyroid-Binding Proteins
Alterations in thyroid-binding proteins can significantly impact the
interpretation of thyroid function tests. In select cases, the
Rockwell approach includes assessment of:
Thyroxine-Binding Globulin (TBG)
Alterations occur with hormonal changes, liver disease, and
certain medications
Transthyretin and Albumin
Changes reflect nutritional status and chronic illness
These measurements help explain discrepancies between free
and total thyroid hormone levels and identify non-thyroidal
influences on test results [12].
